Job Summary: Assist and/or accompany residents transferred to/from the facility to scheduled appointments.
Job Type: FT Schedule: Hours will vary during the week.
Expect to work 30-40 hours a week Responsibilities: Assist residents to enter and exith the transportation vehicle.
Exhiit patience, professionalism, and politeness when communicating with residents, family members, and staff.
Remain with resident during the scheduled appointment.
Report significant changes in resident's condition or poor tolerance for travel to the clinical leadership.
Ensure documentation received while at a scheduled appointment is returned to the nurse upon return to the facility.
Qualifications & Educations: CPR and First Aid Certification are required.
Must possess effective communication skills.
Must be able to physically assist residents (including wheelchair bound residents) on/off transport and in/out of physician offices, etc.
